Can Continuum be my primary care practice?

Yes. We care for new medical concerns and ongoing conditions while working with you on prevention and longer-term health goals. Your primary care and your preventive plan belong in the same physician relationship. You do not need a referral.

Does Continuum accept insurance?

Continuum is a private-pay membership practice and does not accept insurance payments. We can discuss fees and any separate costs when you get in touch.

What happens after I reach out?

We will talk about what you want from your care and help you understand what is possible at Continuum. We can explain the approach, membership options and costs. If you decide to join, we will guide you through enrollment and preparation for your first visit.
