How membership works.

Membership combines concierge primary care with individualized preventive medicine. You have an ongoing relationship with your physician, access to advanced diagnostics and a plan that develops with your health and goals.

Begin with what matters to you.

You may want to understand a family history of disease, improve your fitness or feel better in daily life. You may have a condition that needs closer attention. We start by understanding your priorities, then help you see which options in modern medicine could be useful to you.

A detailed starting point.

Your first assessment includes a thorough medical history, examination and initial testing. Allow about two to three hours. We consider current concerns and family history alongside your activity, nutrition, sleep and emotional health.

When the results are ready, you meet with your physician to understand the findings and build your plan. Together, you choose priorities, discuss treatment and decide how to measure progress. Your baseline gives us something meaningful to compare with as your care continues.

We draw on a range of tests and treatments, chosen around your health and the questions you want to answer.

Advanced diagnostics

Options such as advanced bloodwork, genomic testing and whole-body MRI can add detail to your assessment. Genomic findings may inform questions about inherited risk or response to certain medications. Your physician helps you decide which tests are useful and interprets the findings with you.

Fitness and metabolic health

VO2 max and body composition testing help assess aerobic fitness and changes in muscle and fat. The results can guide practical work on exercise and nutrition, with repeat measurements to see how you respond.

Individualized treatment

Your plan may include medication, hormone therapy or selected peptide therapies alongside work on daily habits. Dr. Schaaf discusses the evidence, potential benefits and risks, including where research is still developing, before you decide on treatment together.

Putting your results together

Laboratory results, health history and genomic findings, when available, are considered together. Analytical tools help us examine patterns and compare results over time, with your physician guiding what changes in your care.

Some testing and services are arranged through outside providers. We discuss what is appropriate for you and any separate costs before you proceed.

Support between visits.

Your relationship with your physician continues between appointments. You can get in touch with concerns or questions about your plan, with follow-up in the office or by telehealth when appropriate.

Between visits, you have practical steps to work on. We review your progress, including changes in your results and how you feel and function. Treatment and goals can change as we learn what works for you. The timing of visits and repeat testing follows your medical needs.
