This policy explains how Continuum Health, LLC collects, uses, and shares information when you visit continuumhealthmd.com or submit a membership inquiry through the public website.
Medical records and patient communications are governed by Continuum's Notice of Privacy Practices and applicable health privacy law. This website policy does not replace that notice.

Information you provide
The membership inquiry form asks for your name, email address, phone number (required if you prefer a call), preferred contact method, how you heard about Continuum, and an optional membership question.
The form is not medical intake. Please do not include medical information. Current patients should use the secure patient portal.
Information collected automatically
Our website and service providers may collect technical information such as your IP address, browser and device type, referring page, pages viewed, approximate location derived from your IP address, and the date and time of your visit.
How we use information

Submitting the form allows Continuum to store your information and contact you about that inquiry. It does not enroll you in a membership or create a physician-patient relationship.

These providers may process information for us under their contracts and privacy terms. We use HubSpot for membership inquiries and Vercel for website hosting. Google Analytics measures use of eligible pages only after you choose to allow it.
We may also disclose information when required by law, to protect rights or safety, in connection with a business transaction, or when you direct or authorize us to do so.
We do not sell information submitted through the inquiry form.
Retention and security
We retain information for as long as reasonably needed for the purposes described above, our business records, and applicable legal obligations. We use reasonable administrative and technical safeguards, but no internet transmission or storage system can be guaranteed completely secure.
Your choices
You may ask about, correct, or request deletion of information you submitted through the public website. Some records may need to be retained for legal, security, or operational reasons. Requests about medical records follow Continuum's Notice of Privacy Practices. Use the contact options in that notice for those requests.
You can keep optional analytics off, change a prior analytics choice in the footer, or use a browser that sends Global Privacy Control.
Third-party services
The website links to services that Continuum does not operate, including the secure patient portal. Their privacy terms apply when you use those services.
Children
The public website is not directed to children under 13. If you are asking about membership for a child or family member, use your own contact information and do not include medical details in the public form.
